I hereby certify that the fryer(s) represented by the model and serial numbers listed above and associated with this Canadian Obsolescence Program, (ID #COP2013) have been destroyed and permanently rendered inoperable for their original intended purpose, or have been converted for use as open fryers only. In the event any of the above fryer(s) are subsequently placed back into operation as pressure fryers, I jointly and severally agree to defend, hold harmless and indemnify Winston Industries, LLC, Louisville, KY, and its subsidiaries and affiliates from any loss or claim, including all attorney fees and costs of defense, arising out of a claim or claims in connection with subsequent use of the fryer(s) described herein, if used as pressure fryer(s).
